PLEASE READ CAREFULLY BEFORE CONTINUING!

EMAIL ADDRESS: The email initially entered for this account will be the primary email used for 4-H communications. Please use an email that you will regularly be checking so that we can keep you up to date.

PASSWORD: Once this enrollment form is completed, you will be sent an automatically generated password. You will be able to log back into your account to change it. 

FAMILY MEMBERSHIP: Those with more than one youth enrolling should select the applicable Family Membership Level. Please enter the oldest youth's information as the first record. Once your account is created with that information, your account will be reviewed and approved. This may take up to TWO DAYS (requires manual approval). Once approved, you will log back into your account using the temporary password received in your email. Once logged in, click the orange bubble in the top right corner, click "view profile" and then "add member". Add a member profile for each of your other children.

INDEPENDENT PROJECT/MEMBER: If you will be checking "Independent Project" and are NOT A MEMBER OF ANY 4-H CLUB OR if you are a member of an existing 4-H club but ALSO want to do an independent project, you must contact the 4-H Program Manager, Emma Jenks at elj57@cornell.edu to discuss requirements.

PAYMENT: When enrolling, payment is required to successfully submit the form. Follow the directions upon completion of the form to take you to the secure payment site. 

RELEASE PACKAGE: Once the 4-H Office has received automatic notification that your membership has been submitted, parents/guardians will receive a fillable electronic version of the "Release Package" which they must complete, sign, and submit electronically. Once the "Release Package" has been submitted and reviewed, the youth's account will be made active. 

VOLUNTEERS: You will enroll and receive your "Release Package" over email as detailed above. The family membership and payment section does not apply to you! 

GOOD NEWS! Although in year 1, lots of input is required, when renewal time comes the following year, you will only have to edit changes, as all fields will remain.
